Complete the alumni request form if you have graduated from Grafton High School and you require a copy of your final transcript for your own records or to be sent to a college/university admissions office. Click on the button below.
Retroactive Diploma Request
Attention Graduates of Grafton High School (2003-2024)
A recent change in the state policy has eliminated the MCAS as a graduation requirement. As a result, students who previously earned a certificate instead of diploma may now apply for a transcript review.If, having met all of the local graduation requirements, you earned a certificate (not a diploma) upon finishing high school, you may request a review of your transcript to see if you are diploma eligible.

Registry of Motor Vehicle (RMV) Documents Request
Complete the License & Permit form to request and receive a copy of your current transcript and a proof of enrollment letter as required by the RMV. Click on the button below.

Documentation requests are processed every Tuesday & Thursday and on Wednesdays during the summer.
Please allow ten business days for all requests.
